Informed Consent for Covid-19

1. To the best of my knowledge, I do not have COVID-19.
2. I am not sick and do not have any symptoms of COVID-19.
3. I have not knowingly been in contact with or exposed to any known carrier of COVID-19 within the past 5 days.
4. I will not knowingly expose others to any illness I may have or be at risk to have.
5. I will follow the Club Guidelines to reduce any exposure and possibility of contracting or spreading the virus.
6. I will follow city, county and state guidelines with respect to COVID-19 that have jurisdiction in the area in which the club event is taking place.
7. If any of the above should change prior to my arrival at the club event or during my participation in the event that is the subject of this consent, I will inform the ride leader or event manager and proceed accordingly in light of the new information.
8. I will notify the ride leader if I develop symptoms or test positive to Covid-19 within the two weeks after a ride.
9. I am attending this Club event entirely at my own choice and assume all risk and full responsibility for my own health, wellbeing and safety.
10. I hereby indemnify and hold harmless the Oxon Hill Bicycle and Trail Club (board members, members and their designees) from all COVID-19 related claims by or liability to any third party arising out of my participation in the Club event. Any liability to anyone for any COVID-19 incident involving myself will be my sole responsibility. I assume all financial liability and assume all and any financial costs associated with my actions.
